Private medical records posted on public website 

A spreadsheet with the confidential medical records for 20,000 Stanford University Hospital emergency room patients, including names and diagnosis codes, was openly posted on a commercial website for almost a year.

The data somehow passed from a hospital billing contractor to a site that sells online tutoring to students.

Until an ex-patient accidentally came across his own records, the information was being used as a sample for a lesson about converting spreadsheets into bar graphs.
