  • Flowers and photos left by mourning fans occupy a memorial to deceased actor Cory Monteith outside the Fairmont Pacific Rim Hotel on July 16, 2013 in Vancouver, British Columbia, Canada. The B.C. Coroners Service released results of Monteith's autopsy today and found the 31-year-old's cause of death was a mixed drug toxicity involving heroin and alcohol.
The body of “Glee” star Cory Monteith has now been cremated, but more information about his last months is coming to light.

An “insider” told TMZ that cast members and producers behind “Glee” knew he had a heroin dependency and tried desperately to get him clean. The group reportedly staged some sort of intervention, to which they say he was “very receptive.”

Monteith checked himself into rehab but only stayed for 30 days, which most addiction experts will tell you is not enough time to fully kick something as insidious as heroin — especially for someone like Monteith, who had been battling substance problems since he was 13. Too sad.
