Fatal oversight: Inspector faked report on park ride that killed boy 

WHAT: A South Carolina licensing inspector is accused of falsifying the preseason report on a county park miniature train that derailed and killed a 6-year-old boy, and sent 12 other passengers to the hospital.

HOW: Twenty-year employee Donnie Carrigan could not have tested the fatal train last week because its battery was dead, making it inoperable. Carrigan marked the train’s running speed as "satisfactory."

WHAT’S BEING DONE: The state’s Department of Labor, Licensing and Regulation fired Carrigan and is still investigating the cause of the derailment.
